Club Event Report Form (CERF)

A Club Event Report Form (CERF) helps the organization record information and
attendance for events. To fill out a CERF, please download and fill out this digital copy,
and email it to ckiucicerfs@gmail.com

CERFs are DUE Every Friday by 11:59PM! Second call for CERFs is due every Sunday by 6pm. If CERFs aren't received then our Secretary, Karen Truong, will be in contact with you soon!

Please title the subject of the email and the CERF file name as "MM.DD.YY ______"
where the event name goes in the blank, and the date goes before it. Thanks for chairing!
